		Instructions for filling out and mailing form:

		1- Click on each field and type in information. The entire form is not visibe so be sure to scroll down

		2- When form is completed click on “file” at top left of your browser then click on “print”


	3- Please make your dues cheque payable to: “EAA Hamilton Chapter 65”. Do NOT send cash in the mail.

		Note: Dues are $50.00 per member per year.


	If you join on or after April 1st dues are pro rated at $5.00 monthly until years’ end.
